The realm of artificial intelligence (AI) has witnessed revolutionary strides in recent years, with the advent of models like the Generative Pre-trained Transformer (GPT) setting new paradigms in natural language processing (NLP) capabilities. Originating from OpenAI, GPT belongs to a class of models termed “transformers,” which leverage attention mechanisms to capture contextual information from input data. Unlike conventional deep learning architectures, such as recurrent neural networks (RNNs) or long short-term memory (LSTM) units that process data sequentially, transformers parallelize the computation, enabling them to efficiently manage longer sequences of data. This parallelization, in turn, allows GPT and its successors to manage vast amounts of textual data, leading to remarkable proficiency in generating coherent and contextually relevant content.
GPT’s distinctiveness lies not just in its transformer architecture but also in its training methodology. As the name suggests, it employs a two-step process: pre-training and fine-tuning. During pre-training, the model is exposed to massive amounts of text, absorbing vast amounts of information, idioms, facts, and even some biases present in the data. This phase enables the model to predict the next word in a sequence, making it proficient in understanding language patterns. Once this unsupervised learning phase is complete, the model undergoes fine-tuning, where it is trained on a narrower, task-specific dataset. However, as groundbreaking as GPT and its iterations like GPT-3 or GPT-4 may be, they also usher in a set of challenges and ethical considerations. Their capability to generate human-like text raises concerns about misuse, such as generating fake news or impersonating genuine human communication. Moreover, since these models learn from vast datasets, they can also inherit and amplify biases present in the data. Thus, while GPT represents a monumental leap in NLP capabilities, its deployment demands caution, transparency, and continuous research to harness its potential responsibly.
Researchers recently delved into how the AI tool ChatGPT can be used in a medical setting, from diagnosing patients to helping doctors decide on treatments. When given certain patient information like symptoms and test results, ChatGPT was able to determine possible illnesses with an accuracy of about 60%. This jumped to almost 77% when more detailed information was available. On average, the AI did best when it was asked to give a final diagnosis based on all available info, scoring about 72%.
However, there were moments when ChatGPT struggled. In some instances, it suggested unneeded tests or could not decide on a clear diagnosis, even when the right answer seemed obvious. In other cases, it did the opposite and completely overlooked the need for useful tests. This suggests that while AI is good at using data to make decisions, it sometimes gets it wrong when the situation is not clear-cut.
Another study explored how ChatGPT was affected by the age or gender of patients. The results found that these factors did not seem to skew the AI’s accuracy, but the researchers believe it is worth looking into other potential biases in the future. It is important to note that ChatGPT makes decisions based on predicting the next best response, rather than real reasoning. This means it can sometimes make errors that could be serious in a medical context, like suggesting the wrong dosage of a medication.
AI’s getting smart. We have seen it help in identifying problems like skin cancer and Alzheimer’s by looking at clinical images. In places where patients may not have access to as many specialist doctors as in other locations, AI might fill a gap. The GPT-4 program, for instance, can look over a patient’s medical history and guess what might be wrong with them. A study was done to see if GPT-4 could help doctors get their diagnosis right, especially in tricky situations.
How did the study work? They took the medical records of six patients who had confusing health issues. They fed these records into GPT-4 without revealing the final diagnosis. They then compared what GPT-4 guessed with what the doctors guessed. Another tool, Isabel DDx Companion, was also assessed in the study. Out of the six patients, GPT-4 got the main diagnosis right for four of them (that is about 67%). The doctors got it right for just two patients (33%), and the Isabel tool did not get any primary diagnoses right. When they included other diagnoses, the accuracy increased for all, but GPT-4 was still ahead in terms of levels of accuracy and outcome. Interestingly, certain words seemed to help GPT-4 make its guess, and in some cases, it even thought of diagnoses that the doctors did not.
GPT-4 could be handy, especially for patients who have not been treated or seen by their physician in more than a month. Again, it can be especially useful in places where specialist access is limited or rare. But there is a catch. GPT-4 needs detailed info to work right and sometimes its suggestions might be a bit off. It is also not perfect in spotting some specific problems. This does not mean AI is ready to replace doctors. It is promising, sure, but there are bumps to iron out. AI is more like a new assistant in training – helpful but not yet an expert in the craft.
Remember the 90s? That is when “robot therapists” first popped up, using scripted advice. Today, there are apps that use advanced AI to chat about your worries. ChatGPT, though, is different in terms of how it might interact with a patient. It is more human-like because it is trained using vast amounts of online text. In addition to patient treatment options, it can even help therapists by managing paperwork, which in turn allows them to spend more time with patients. However, using ChatGPT alone as your personal therapist? That is complicated. Some believe it can sometimes give better feedback than humans but bridging the gap between possibilities and patient acceptance might take more time. For instance, researchers at the University of Washington have made a tool where you type in sad thoughts, and the program helps you see the bright side and potentially overcome your sad thoughts. Over 50,000 people tried it out and loved it more than other similar tools.
With the global shortage of mental health experts, as well as insurance plan limitations on mental health benefits, chatbots like ChatGPT might offer some solutions. Thomas Insel, an American neuroscientist, psychiatrist, entrepreneur, and author who led the National Institute of Mental Health from 2002 until November 2015, says chatbots can be particularly useful in this field because mental health is all about conversation. As these tools develop and proliferate, there are concerns about privacy, the quality of advice from chatbots, and how tech companies will make these products responsive to a wide enough spectrum of patients and their conditions.
There is also a debate about how these chatbots should be regulated. If they are not checked, they might do more harm than good. Some companies are trying to get approval for their chatbots to be considered medical devices. But without clear rules, it is up to users to decide if a chatbot is trustworthy. The big question? Is some therapy, even if from a chatbot, better than none? Given the huge demand for mental health support, chatbots might just be a part of the solution.
